Admission Policy

Saint Rose will consider all referrals made for entry into the program. Saint Rose will not discriminate in the admission to and the provision of needed services to recipients on the basis of age, race, color, sex, religion, national origin, ancestry or handicapping condition.  

Admission into the Developmental Training Program is based upon an assessment of the client's adaptive and maladaptive behaviors as outlined in the client exit/entry criteria (Developmental Training Proposed Rules, sec. 119.205).  There is no minimum funcationing level required to enter the Developmental Training Program.  Clients who have met the exit criteria for D.T. will not be enrolled.  The client's interdisciplinary team is responsible for conducting or supervising the assessment process.  The interdisciplinary team includes the following: The Director of Developmental Training, Developmental Instructor, Developmental QMRP, Client, Client’s family/guardian. Assessments will be conducted to identify self help, cognitive and independent living skills, physical and motor skills, intellectual capacity, psychological and emotional stability, and social and developmental history.  The ID team will make the final determination as to the level and appropriateness of day programming.  If the ID team approves, prospective recipient is offered a 30-day trial period in the Developmental Training Program.  To be followed by an admission staffing.

  • Copy of insurance Card-Medicaid/Medicare/other
  • Copy of Guardianship Papers
  • Copy of SSI/SSDI Public Aid Documentation
  • Psychological
  • Medical Records
  • Dental Records
  • Progress Reports from prior school/agency of attendance
  • Current income tax return of guardian/family and/or person to be served
  • Birth Certificate
  • Baptism/confirmation documents
  • Selective service (males only) documentation

Saint Rose works with the Department of Human Services to provide funding for all who attend and utilize the services it has to offer. Individuals to be serviced will be asked for a service fee according to the family’s ability to pay.

Waiting List or inappropriate program placement.

Upon acceptance prospective recipients have priority to fill slots that are opened.

If no slots are available, they will be referred to other organizations that can meet their needs or placed on a waiting list. If it is determined that the costs the organization would incur to make the needed changes for a given individual are excessive, the person would be referred to another resource. If resources are not readily available the organization will document the steps it has taken and the organization may deny admission.

Prospective recipients exhibiting self-injurious behavior, requiring daily psychiatric intervention or ongoing medical intervention or pose a real risk in our providing services will be assisted in finding a suitable facility.
